Thank you for your interest in updating your provider profile on SutterHealth.org. A complete and accurate online profile helps patients find you more easily and better understand your expertise, leading to stronger connections and more informed care.
This form lets you update certain profile details - such as your scope of practice, philosophy of care, languages you speak, highlight community activities and more.
Please note: Some fields must go through your local credentialing or medical staff team before changes can appear on SutterHealth.org. These include:
- Name or credentials
- Specialty and practice status
- Address, phone or fax number
- Education, or certifications
- Hospital/facility affiliations
If your update involves any of these, please contact:

- Or your local medical staff team (hospital, surgery center, etc.)
Important: Don't include any Protected Health Information (PHI).
